Registration
In order to guarantee the best possible experience for all players, we will accept a maximum of 10 team entries.
Depending on demand, second (and subsequent) team entries from individual clubs may be put on hold (fully refundable if not ultimately accepted) to ensure that all clubs have a chance to enter at least one team. Entries that were put on hold will be accepted in the order in which they were received.

Fees
- $800 per mixed team, maximum 15 players per team, maximum 12 suited per game
- $200 per women's team.
- $25/person for the banquet
- After October 22, 2021: $900 per mixed team, $250 per women's team, and $35/person for the banquet

Teams
- All teams must be US teams and players must be residents of United States.
- All players must be registered through USOA.
- Teams competing for the National title must be comprised of players registered through USOA with only one guest player (not your designated team on USOA) is allowed per team competing for the national title.
- Teams without enough players are allowed to combine so long as they players are US residents and understand that, if they win, they understand they will not be recognized as the national champion.
- If a club would like to bring two mixed teams, that is great! Only one of the club's teams will be eligible to win the title of National Champion.

Payments must be received by October 22 to avoid any penalties.

Registration Process
- Secure your team's spot at the 2021 National Championships by sending payment by October 22, 2021:

USOA
All players must be members of USOA Underwater Hockey. No money or waivers will be accepted at the pool. All USOA fees and waivers must be submitted by October 22, 2021. We will obtain the list of paid dues and signed waivers to cross-check with our rosters on that date, and anyone who has not completed this task WILL NOT BE PERMITTED TO PLAY.
